OUR PATIENTS AND FAMILIES
You and your family's participation is critical to the success of the care we provide. We encourage you to actively voice concerns and opinions to your physician and our staff. There may be times when difficult decisions concerning you or your loved ones care will need to be made. Our team will be there to help you understand the facts about your condition and the different treatment options available.
Hospice Services are provided to you and your family based upon your individual needs. Not all services are needed, or even wanted, by every hospice patient. The Missouri River Hospice Team works closely with you and your physician to develop what is known as a Plan of Care.
This plan helps direct our staff in providing services and helping you meet the various goals of your care. A plan of care is dynamic in that it changes as your individual and family needs change over time. Although a plan of care is customized to your individual needs, the following pages summarize the key elements of every care plan:
